“Donate Life, Pass It On”
Customized License Plates

New Yorkers can express their support of organ and tissue donation through the purchase of a customized New York State “Donate Life” license plate.  By doing so, New Yorkers can also contribute to funding needed for public education about organ and tissue donation. 

The plate incorporates the “Donate Life” logo and promotes organ and tissue donation in New York State.  State residents can order the plate online at the Department of Motor Vehicles website or by downloading and printing an order form.

Twenty dollars of the annual fee for the plate will go towards the “Life Pass It On” trust fund, which provides funding for public education about organ and tissue donation.  The fund is administered by the Commissioner of Health and is reserved for donation awareness events, projects and activities in New York State.  New Yorkers can also make a voluntary $1 contribution to the “Life Pass It On” fund when applying for a driver license original, duplicate, amendment or renewal.
